Enrollment Immunization Requirements

As a condition of enrollment, the California State University (CSU) requires students enrolled or taking courses at CSU campuses to provide proof of immunization against measles, rubella and hepatitis B (CSU Executive Order 803).

Requirement Overview

As a condition of enrollment, the California State University (CSU) requires students enrolled or taking courses at CSU campuses to provide proof of immunization against measles, rubella and hepatitis B (CSU Executive Order 803). In addition, certain students must complete tuberculosis screening and acknowledge receipt of meningococcal disease information. Please carefully read the following information for details on the health requirements that relate to you.

Per Executive Order 803, students who do not provide proof of full immunization by the beginning of the second year of enrollment are not allowed to enroll until proof of full immunization has been provided to the Student Health Center.

Immunization Requirements for All Students

Measles and Rubella Immunization

All students born after January 1, 1957, are required to present proof of measles and rubella immunization to the Student Health Center prior to their first enrollment. Students must complete this requirement by the beginning of the second year of enrollment.

Exemption Notice

  • Students enrolled in a California public school for the seventh grade or higher on or after July 1, 1999, will have satisfied the measles, rubella and hepatitis B requirement.

  • Students who will be enrolled only in courses through distance learning should contact the Office of the Registrar.
  • Medical considerations may also exempt certain students from this requirement. Please schedule an appointment with the Student Health Center.
    • Note: A written statement by a physician or surgeon that the physical condition of the person or medical circumstances relating to the person are such that immunization is not considered safe, indicating the specific nature and probable duration of the medical condition or circumstances that contraindicate immunization is required.

Hepatitis B Immunization

All first-time enrollees who are eighteen years of age or younger are required to present proof of immunization against hepatitis B prior to their first enrollment. Students who enroll who are partially immunized against hepatitis B must provide proof of full immunization by the beginning of the second year of enrollment.

Additional Requirements

International Students

In addition to the measles, rubella and hepatitis B immunization requirement, all international students are required to provide the Student Health Center with verification of a negative tuberculosis (TB) skin test, chest x-ray, or QuantiFERON test taken within the last year.

Immunization and freedom from tuberculosis verification must be submitted on the Health Requirements for Registration Clearance form the first day of the first semester of attendance. Note: Please take this form to your doctor for completion before leaving your country.

On-Campus Housing Residents

Housing residents have two additional health requirements which must be completed prior to move in. For additional information, click on Housing and Residence Life or contact the Student Health Center.

TB Clearance

Housing residents must provide proof they are free of tuberculosis (TB) or that they are not at high risk for tuberculosis and require a TB test. Prospective residents must complete and submit to the Student Health Center the Tuberculosis (TB) Risk Assessment.

  • Students who require TB screening may complete the screening through the:
    • Personal healthcare provider of their choice
    • Student Health Center
      • An appointment is required for TB screenings. Students may call 323.343.3300 to schedule appointments.

Meningococcal Disease

Incoming freshmen residents must also read, sign and return the Off to College meningococcal disease fact sheet to Housing and Residence Life.

Submission of Acceptable Proof

Submission Options

Students who have acceptable proof of immunization and negative TB screening may mail, FAX or hand carry verifications to the Student Health Center.
